Traveling Consultant Pharmacist (PACE Program)

Fort Gratiot, MI

Full-Time | Weekdays

$130K-$150K


Overview

We are seeking a compassionate and clinically strong Traveling Consultant Pharmacist to join our team in supporting a PACE (Program of All-Inclusive Care for the Elderly) program in Fort Gratiot, MI. This role is ideal for a pharmacist passionate about geriatric care and interdisciplinary, patient-centered practice.


As a key member of the care team, you will help optimize medication therapy and improve health outcomes for older adults with complex medical needs. 


Key Responsibilities

  • Review and manage medication regimens for PACE participants
  • Conduct medication reconcilliations and detailed chart reviews
  • Identify, resolve, and prevent medication-related problems
  • Particpate in interdisciplinary team (IDT) meetings and care planning
  • Ensure compliance with CMS regulations, state pharmacy laws, and PACE standards
  • Provide education to participants, caregivers, and healthcare team members


Qualifications

  • Doctor of Pharmacy (PharmD) from an accredited program
  • Active Pharmacist License (required)
  • Active Controlled Substance License (required)
  • 2+ years of experience in geriatrics, long-term care (LTC), or related setting
  • PACE experience preferred
  • Willingness to travel
