Job Description

Role Overview
The Care Coordinator conducts timely assessments for elderly and frail clients in community settings, develops and maintains comprehensive care plans, provides ongoing case management and referrals, ensures accurate documentation and certifications, collaborates with internal teams, travels within assigned communities, and engages in continuous learning to support high‑quality client care.
 
 
Job Responsibilities
  • Engage and assess the complex needs of elderly, frail and clients in community settings. Gather information, develops care plans and connects clients to other resources and supports.
  • Conduct assessments for new and existing program participants within established timelines. Provide case management services. Develop well written and effective care plans which appropriately match the needs of the client. Maintain care plans and client confidentiality.
  • Make appropriate referrals, monitors client services, documents services provided within established timelines. Maintain up-to-date client files and databases. Attend required meeting and trainings. Maintain mandatory certification.
  • Work collaboratively with internal stakeholders and team members as needed.
  • Required to travel independently throughout assigned communities to complete position duties
  • Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

 

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.

  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day
  • Climb up and down up to 3 flights of stairs at a time
  • Lift, push, and pull up to 25 lbs

Education and Experience Requirements:

Minimum
  • Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study Entry level
  • Organization, time management, client/caseload management skills and the ability to maintain confidential client material.
  • Strong communication, problem solving and critical thinking skills needed.
  • Data entry skills required and CP software knowledge required.
Preferred
  • Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 1-3 years relevant experience
  • Care Coordination/ Case Management Experience.
  • Experience working in aging network, federal waiver program or managed care. Ability to build relationships with diverse populations.
  • Strong communication and interpersonal skills preferred.
 
Certifications and Licensure
  • Minimum IDOA Certification upon hire
