GlenBrook Health Center is looking for a Part-Time MDS Coordinator! The team member will promote the physical and emotional well-being of nursing facility residents. They will gather information from residents and their families and the interdisciplinary care team during initial and periodic interviews. The core responsibility of the MDS Coordinator is to conduct patient assessments and interviews, coordinate and verify accurate and timely completion of MDS�s and CAAS. Information from these assessments are used to formulate baseline and individualized care plans in collaboration with the interdisciplinary care team.
- $52 - $55/hr. (DOE)
- Part-time: preferably 8-10 hours per week

What you will bring:
� CA Licensed RN or LVN with a current license in good standing.
� Minimum 2 years� experience as a MDS Coordinator in a Medicare Skilled Nursing Facility including MDS 3.0 experience preferred
� Current CPR certification
� Must have strong knowledge and working experience with State and Federal regulations, the RAI (Resident Assessment Instrument) and working knowledge of PDPM criteria.
� Computer Proficient with Microsoft Word, Outlook and EMR system, preferably Point Click Care.
� Current certification as Resident Assessment Coordinator (RAC-CT)
� Must demonstrate detail orientation, capable of directing others and managing assessment schedules to maintain compliance.
� Excellent communication skills required.
� Flexible, decisive and responds quickly in high-pressure situations and completing work in a timely, accurate manner.
