The Resident Care Coordinator is responsible for scheduling, training, and mentoring caregiver and med tech personnel to deliver clinical care and within scope of practice and coordinates care delivery, which ensures resident’s needs are met in accordance with professional standards of practice through physician orders, facility policies and procedures, and federal, state and local guidelines.  As a member of the Health & Wellness team, you ensure industry-leading resident care.

Requirements

Education: High school diploma preferred; High school equivalent required

Skills: Basic reading and writing skills, and ability to speak English in an understandable manner
Years of Experience: 2-3 years resident care experience and training staff required; prior experience with dementia population preferred
Licensure: Current active LPN license in state of practice strongly preferred & required for Sign On Bonus

Physical Demands: Ability to stand, walk, bend, and squat for prolonged periods; Ability to push and pull objects and lift and carry up to 50 pounds, unassisted, on a frequent basis; Full use of hands, arms, and legs (for writing and medication administration); Ability to read thermometers, charts and testing equipment
