What You�ll Do

As a Home Health LPN, you�ll provide hands-on, compassionate care while working closely with an interdisciplinary team:

  • Deliver direct patient care in accordance with the state Nurse Practice Act
  • Carry out care plans developed by the Registered Nurse
  • Educate patients and families on self-care and treatment plans
  • Document care accurately and timely
  • Coordinate with RN and care team to ensure patients receive comprehensive support
  • Utilize proper techniques for equipment, supplies, and infection control
  • Support patients� physical, emotional, and educational needs in the home

What You�ll Need

  • Active LPN/LVN license in the state
  • Graduate of an accredited practical/vocational nursing program
  • 3+ years of nursing experience (Home Health, Med-Surg, or Community Health preferred)
  • Current CPR certification
  • Reliable transportation, valid driver�s license, and auto insurance
