TITLE: Assisted Living Activity Coordinator

REPORTS TO: Housing Manager

Job Summary: To plan for and provide activities to residents; enhancing and enriching the emotional, spiritual, social, and physical lives of each individual based on the comprehensive resident assessment. Designs activities compatible with resident’s known interests, needs, abilities and preferences.  The goal of the Activity Coordinator is to support the highest level of independence for each resident, being able to identify both strengths and deficits.  Engages residents in activities of interest and promotes quality of life for each resident. Provides leadership and guidance to the employees, if any, within the Activity Department.

Qualifications: Must be able to read and speak the English language. Prefer to be a qualified activities professional who is recognized by an accrediting body.  And/or has completed a training course approved by the State and has had at least two years’ experience in an activity program. Dementia training preferred.  Complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

General Responsibilities:

  • Demonstrates dependable, professional work habits.
  • Develops monthly activity calendar for Assisted Living Facility.
  • Assists in marketing by taking photos of regularly scheduled events and special occasions.
  • Acquire, store and maintain activity supplies.
  • Plans and attends programs for holidays and special events.
  • Attends Quality Assurance and staff meetings as needed.
  • Participates in staff and community education as needed.
  • Complete required documentation for all residents in a timely manner.
  • Cultivate and maintain positive relations with residents, families and the community.
  • Develop and maintain effective communication & cooperation with departmental personnel, as well as all other departments within the facility to ensure that services and activities can be completed to meet the needs of the residents.
  • Direct the development, implementation, supervision and ongoing evaluation of the activity program.
  • Complete the activities component of the comprehensive assessment and contribute to comprehensive care plan goals and approaches that are individualized to match the skills, abilities and preferences of each resident.
  • Maintain a clean environment for the resident. Follow proper infection control practices in all aspects of work.
  • Must understand and practice Standard Precautions.
  • Recognize safety responsibilities of employee for the resident.
  • Report occupational hazards and/or incidents of injury, to appropriate personnel, in accordance with facility policy and procedures.
  • Recognize employee’s responsibility for his / her own safety while on the job and for the safety of others. Recognize and remove environmental dangers.
  • Apply principles of good body mechanics when lifting and moving objects. Use transfer techniques consistent with proper body mechanics.
  • Participate in disaster planning drills (fire, tornado, severe weather, etc.) when on duty.
  • Maintain awareness and adheres to use of safety devices according to the care plan and suggests their use when appropriate.
  • Develop knowledge of actions to be implemented in emergency situations.
  • Practice fire safety and accident prevention procedures.
  • Follow all policies and procedures governing the operation of Minnewaska Community Health Services.
  • Must understand and adhere to all Vulnerable Adult Abuse Prevention Policies.
  • Adheres to HIPAA privacy rules and regulations.
  • Must obtain 4 hours Dementia Training within 160 hours of start of employment.
  • Other duties as assigned by Housing Manager.
