Registered Nurse (RN) for Home Health Care Responsibilities:

• Provide high-quality, patient-centered care to patients in their homes, focusing on symptom management, education, and support

• Conduct thorough assessments, develop individualized care plans, and implement interventions to promote optimal health and wellness

• Collaborate with patients, families, and interdisciplinary team members to ensure comprehensive care

• Maintain accurate and timely documentation of patient care and treatment outcomes

• Participate in quality improvement initiatives and contribute to the development of best practices in home health care

• Stay current with industry trends, research, and regulatory requirements to ensure compliance and excellence in care

Registered Nurse (RN) for Home Health Care Requirements:

• Current licensure as a Registered Nurse (RN) in the state of Pennsylvania

• Minimum one year experience in home health care or a related field

• Experience with completing the OASIS assessment strongly preferred

• Strong clinical skills and knowledge of adult and geriatric care

• Excellent communication, interpersonal, and organizational skills

• Ability to work independently and as part of a team

• Reliable transportation and ability to travel to patient appointments
