Greenfield Rehabilitation Agency is seeking a PRN Physical Therapist (PT) to support our newly acquired senior campuses in Pigeon Falls and Whitehall, WI. This is an excellent opportunity for a dedicated and flexible PT who wants to make a meaningful impact while working within a supportive, therapist-led organization.

Your Responsibilities:
Perform evaluations and provide skilled physical therapy treatment in a skilled nursing setting
Develop individualized care plans to improve mobility, strength, balance, and overall function
Collaborate with the interdisciplinary team to ensure continuity of care
Complete documentation in accordance with regulatory and company standards
What You Bring:
Active Wisconsin Physical Therapist license
Strong clinical reasoning, communication, and organizational skills
A passion for working with older adults in long-term care environments
Dependability, flexibility, and a team-oriented mindset

Qualifications
- Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
