Your Role
Provide compassionate, high-quality care to residents in our skilled nursing facility
Administer medications and treatments as prescribed and within your LPN scope of practice
Collaborate with RNs, CNAs, and other team members to ensure coordinated, person-centered care
Monitor residents’ conditions and promptly report changes to the charge nurse or Director of Nursing
Promote restorative care to encourage residents’ independence and well-being
Maintain accurate and complete documentation of all nursing care provided
Support and assist fellow nursing staff to maintain a positive, patient-centered environment
Ensure resident comfort, safety, and dignity while upholding facility standards
Who We’re Looking For
Current Licensed Practical Nurse (LPN) license in the state of Illinois
Graduate of an accredited nursing program
Compassionate and committed to delivering quality, personalized care
Team-oriented, reliable, and professional
