Minimum Requirements:
- High school diploma or GED
- Ability to meet criminal background and drug screening standards (NO Felony, NO pending Court dates)
- Must be 23 years or older
- Must have valid drivers license
- Must be able to speak, read and write in English language.
Preferred Education and experience
- Preferred active (SRNA) State Registered Nursing Aide license
- Preferred at least 1 year experience working with IDD population
Details About the Job
The Hazelwood Center is a Intermediate Care Facility that supports the long term care needs of individuals with intellectual disabilities. The Direct Support Professional position is vital to safety, happiness, and wellbeing of our clients. The support level required will be based on each client’s individual needs. In this role you will provide assistance directly to clients and Nursing department.
Essential Functions
- Responsible for taking/ driving individuals to medical appointments in company provided van.
- Assist with in-house specialty clinics
- Ensure paperwork for outside medical appointments is placed on unit medical calendars.
- Upkeep of floor charts
- Organizing nursing section of clients Lifebook monthly
- Stocking nursing items
- Ensuring total fluid intakes are correct
- Other job duties as assigned
- Attends in-service training programs and participates in drills for and responds to facility emergencies.
- Learn and follow all policies and procedures
Position Type/Expected Hours of Work
This is a full-time position 37.5 hours typical hours are 8AM-4:30PM Monday - Friday, Must be able to flex hours to meet client appointment needs. Employees must be able to work at least 7.5 hours per day with no restrictions to meet the needs of the facility.
