Job Description

Patient Navigator provides direct support services to client, including by not limited to accompaniments to appointments, treatment adherence support, facilitation of client education groups and outreach activities, client assistants around management of medical appointments and other social services appointments, case management support to program team.



 



Help People Living with HIV Stay Connected to Life-Saving Care



We are seeking a compassionate and organized Patient Navigator to support individuals living with HIV in accessing and staying engaged in care.



This role is part of a Ryan White–funded HIV care program focused on improving health outcomes for people living with HIV (PLWH). The Patient Navigator plays a critical role in helping clients overcome barriers to care, remain engaged in treatment, and achieve viral suppression.



 



Working within a harm reduction, client-centered framework, this role helps clients overcome barriers, navigate healthcare systems, and achieve improved health outcomes, including viral suppression.







Key Responsibilities




  • Link clients to HIV primary care and support ongoing engagement

  • Monitor appointments and conduct outreach for missed visits

  • Support ART adherence and viral suppression goals

  • Assist with scheduling medical, behavioral health, and substance use services

  • Coordinate transportation, referrals, and social services (housing, benefits, food access)

  • Provide overdose prevention education and naloxone training

  • Maintain accurate documentation in line with Ryan White standards

  • Collaborate with medical providers and multidisciplinary teams.
