We are seeking a skilled and reliable Registered Nurse (RN) to provide Wound VAC management and skilled nursing support for one of our home health patients. This role requires an RN with strong wound care expertise, excellent documentation skills, and the ability to work independently while maintaining patient comfort and safety. Wound Vac Care is needed Wednesdays and Thursdays.
Key Responsibilities
- Perform Wound VAC changes, assessments, and maintenance in accordance with physician orders and agency protocols.
- Conduct comprehensive wound assessments, monitor healing progress, and report changes promptly.
- Provide patient and caregiver education on wound care, infection prevention, and overall health management.
- Document all visits in a timely, accurate manner using our EMR system.
- Communicate effectively with the care team, physician, and agency leadership to ensure continuity of care.
-Maintain compliance with CHAP standards, state regulations, and agency policies.
Qualifications
- Active Pennsylvania RN license in good standing.
- Experience with Wound VAC therapy is required
- Prior home health or wound care nursing experience strongly preferred.
- Strong clinical judgment, time management, and communication skills.
- Must be dependable and available every Wednesday and Friday for scheduled visits.
