We're seeking a compassionate, experienced Nurse Practitioner to provide telehealth primary care services with a focus on addressing social determinants of health (SDOH). You'll conduct clinical assessments that unlock access to Community Health Worker (CHW) services, helping patients overcome barriers to care and achieve better health outcomes while reducing emergency department reliance.

Duties and Responsibilities:
- Conduct telehealth visits (typically 99202/99203 level) for Medicaid patients seeking support with SDOH challenges
- Perform comprehensive clinical assessments to identify health needs and social barriers to care
- Provide medical clearance and referrals for CHW services to address access issues, home equipment needs, social supports, and care coordination
- Develop individualized care plans that address both clinical and social determinants of health
- Collaborate closely with Community Health Workers to support patient success
- Manage prescription refills and ongoing primary care needs as relationships develop
- Document visits thoroughly to meet state Medicaid program requirements
- Help patients navigate complex healthcare systems and connect to community resources
Minimum Qualifications
- Active Nurse Practitioner license with prescriptive authority
- Primary care experience in the state of CA, OK and HI (Family Practice, Internal Medicine, or similar)
- Experience working with Medicaid populations
- Strong understanding of social determinants of health and their impact on patient outcomes
- Comfortable with telehealth platforms and remote patient care
- Excellent communication skills and ability to build rapport virtually
- Knowledge of state Medicaid requirements for CHW referrals
