Job Summary:
The Assistant Director of Health Homes provides assistance in delivering administrative supervision, management and support to the health home program director and program staff to ensure that quality programming and services are provided to meet the medical, behavioral, social, and developmental needs of the members served in the Health Home program. The AD will oversee staff in both the Bronx and Brooklyn offices and should be flexible to travel to each office.
*Primary location of this position can be the Bronx or Brooklyn.
Essential Tasks:
Assist the director with monitoring census and enrollment of members into Health Homes for program development.
Provide broad support and impromptu individual and group supervision to Health Home Care Manager (HHCM) supervisors.
Monitor service provision to ensure billing occurs.
Maintain up to date knowledge of Health Homes, Children and Family Treatment Support Services (CFTSS), Home and Community Based services (HCBS), and Managed Care.
Maintain current awareness of DOH regulations
Discuss operational issues as advised by the director or BHW-AVP; update staff on new or changed regulations and review electronic health records (EHR) and documentation to ensure regulatory and in-house compliance.
Assist the director in managing and completing audits as needed.
Assist the director with working closely with Program Quality and Compliance departments to enhance service provision, promote better outcomes, and monitor the overall quality management of the program.
Collaborate with the Director to coordinate staff development activities and in-service trainings.
Oversee training compliance.
Facilitate, manage, and organize the IR program committees.
Supervise the program support specialist.
Manage wraparound expenses.
Attend lead health homes (CBC, CCF) weekly virtual office hours and monthly meetings
Qualifications:
- A Master’s Degree in Social Work, Psychology, Public Administration, Health Care or hospital administration with a minimum of two years of relevant experience. Relevant experience must include supervisory experience in a mental health program and/or medical health program, providing service coordination and information, linkages, and referrals for community-based services to children with special needs, complex trauma, and/or individuals with disabilities.

Salary: $80K
