The District sincerely appreciates your willingness to support programs at Lake Park High School District 108. This posting is for use by those interested in being an Event Worker at Lake Park High School. Please be aware that should the event you are assigned to work be canceled, no pay will be issued.
The School Code requires applicants seeking employment at a school in a position requiring direct involvement with children or students to authorize an employment history review as part of the application process. To meet this requirement, please follow the steps below:
Step 1: Create Your Account
- Visit www.Verifent.com
- Click the Get Started button, then Login
- If you don't have login credentials, click Register under the Employee Heading
- Create your account/profile
Step 2: Request Employment Verification
- On the left menu bar, click Request EV
- Type Lake Park and select Lake Park Community High School District 108 from the drop-down list
- Click Save Hiring Entity
Step 3: Select Verification Type
- For the Experience Verification Workflow, check the box for Faith's Law Employment History Review (EHR)
- Do NOT check the other two boxes
Step 4: Complete Former Employers Section (if applicable). If you have a former paid employer for whom you worked directly with children or students, complete the Former Employers section.
Step 5: Authorization & Self-Disclosure
- Complete your Authorization & Self-Disclosure Questions
- Electronically sign your name
- Click I Agree - Continue
Step 6: Confirm Your Order
- Click Next to confirm your order
- You will NOT need to pay a fee (because you entered Lake Park as your hiring entity)

Required Documents
Please note that all required documents listed below (paper and electronic) must be on file in the Human Resources Office before you may begin working. Event Workers are prohibited from beginning any work at Lake Park until after they receive written confirmation from the program coach or coordinator. Please download, complete, and bring the required forms to your fingerprinting appointment.
1) Lake Park High School District 108 Online Application - Complete and submit online.
2) W-4 Forms (Federal and State) - Download Federal W-4 | State W-4
3) Direct Deposit Form – Download form. As your required backup document, please include a blank check with “VOID” written on it, OR a Direct Deposit Verification form from your bank that lists your account and direct deposit routing number.
4) DCFS (Department of Children and Family Services) Background Check - Download DCFS form (print neatly and include your middle name, if you have one). PLEASE NOTE: The DCFS background check process has been updated. You (or your parent or guardian if you are under 18) must complete additional information on the DCFS website. Details will be emailed to you (or your parent or guardian) from DCFS once we submit initial information to them.
- If you are under 18, please list your parent or guardian's personal email address on the form--NOT your own.
- You, or your parent or guardian (if you are under 18): Complete the DCFS online process immediately upon receiving the email from the DCFS.
5) I-9 Employment Eligibility Verification - Download I-9 form. Complete Section 1 on page 1 only--our Human Resources office completes the rest of the form. Bring the following to your fingerprinting appointment:
- Social Security Card (required for payroll)
- One valid government-issued, photo ID (Driver’s License, State ID, or US passport).
6) TalentEd Records Electronic Notification/Acknowledgements - Complete electronically for:
- Board Policy
- Child Abuse/Mandated Reporter
- Guidelines for Acceptable Use of Technology System
- Professional Boundaries
- Emergency, Illness, Accident form (to be updated annually)
Login information for completing these electronic documents will be given once you have submitted your online application and the Human Resources Office receives notification from the program coach or coordinator that you will be working.
7) Criminal Background/Fingerprinting (CBI) - Required for all applicants. Fingerprinting is done by appointment only at: Lake Park High School District 108 Administration Center, 590 S. Medinah Road, Roselle, IL. Please contact Terri Svendsen with Human Resources at 630-295-5421 to set up an appointment to be fingerprinted and to drop off your required documents.
SALARY/RATE: Daily (per event) rate of $70.38 to $191.36 per Appendix F (Special Services) of the Licensed Salary Schedule in the District 108 Negotiated Agreement.
