Overview

Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Located in Proctor, Diamond Willow is dedicated to providing quality care to our 56 residents through compassionate care and ensuring that our residents and their needs remain at the forefront of our focus.





Diamond Willow has partnered with Aspire Clinical Intelligence (a clinical on-call help center for care staff) to help support our ongoing clinical needs when our Registered Nurses are off-site. This means more time for our RN’s to recharge and give back to our residents and staff.  Imagine doing what you love but still getting the needed time off.

Key Responsibilities:

•    Provide on-site supervision and guidance to unlicensed personnel (ULPs) 

•    Assist with initial onboarding competency training and ongoing staff education 

•    Ensure accurate and timely medication check-in and reconciliation from pharmacy deliveries 

•    Support staffing needs, including direct care assistance on the floor as needed 

•    Complete comprehensive and focused nursing assessments as directed by the Clinical Nurse Supervisor 

•    Communicate with families regarding resident updates, concerns, and care needs as appropriate 

•    Assist with supply inventory and organization 

•    Support compliance efforts, including participation in safety and emergency preparedness drills 

•    Assist with tours for prospective residents and families 

•    Support admissions and readmissions to ensure smooth transitions and regulatory compliance 

 



 



 



Qualifications:

•    Active RN license in the state of Minnesota 

•    Experience in assisted living, memory care, or long-term care preferred 

•    Strong leadership, communication, and organizational skills 

•    Ability to work independently while collaborating with interdisciplinary team members

Holidays:  Six holidays are observed during which employees are not required to work.
