Job Description

  • Tracking preventative care requests. Works with members and Care Managers to connect members to preventative care providers and schedule appointments
  • Registering members for vaccine appointments in community and home
  • Troubleshooting issues member experience with medical providers and appointments
  • Coordinating care management mailings
  • Provides administration and department support for care management team. Assists in completing activities related to different projects/initiatives in support of departmental goals.
  • Maintains and supports documentation, logs and data entry to monitor activity and/or outcomes in support of departmental goals as needed.
  • Back-up to the Member Service/Transportation department and offer coverage/assistance as needed.
  • Performs other duties as assigned.

Specific Knowledge, Skills, and Abilities:

  • Proficient in the use of office equipment and the computer; Microsoft Word and Excel.
  • Self-directing, good organizational and interpersonal skills.

Required Education, Experience, and Licenses:

• Graduate of high school
• 1 year satisfactory office experience; experience in Health Care, desired.
