Job Description/Requirements

Job type: Part time

Location: This is a Home Health CNA/CHHA Position - One on One Care in the Clients Home located in Chino

Start : Immediately  

Care Unlimited Health Services seeks a Certified Nurses Assistance (CNA)  to work as an integral part of our Home Healthcare team.  In this role, he/she will provide direct and continuous care to the patient according to the medical plan of treatment and nursing care plan.  

Qualifications

•          Candidate must possess good assessment skills and also have an empathetic personality

•          Basic documentation with computer skills, must have the ability to travel to the patient’s home

•          Current California State CNA Licensure, CPR Certificate, Negative TB Test Result Documentation

Responsibilities

•          Provide shift care in accordance with the written plan of treatment 

•          Document in accordance with Care Unlimited guidelines and established time frames

•          Monitor and report changes in patient symptoms and behavior
