Job Description

POSITION SUMMARY

Obtains medical histories and performs complete and/or appropriate examination, orders laboratory tests, x-rays and treatment programs under the supervision of a staff physician in an assigned area for patients of all ages. The advanced practice medical provider will be credentialed by the CHC/SEIA Board of Directors based upon the CMO’s recommendation and the scope of services.

ESSENTIAL FUNCTIONS/DUTIES

·         Provides direct medical care in collaboration with staff physicians; including but not limited to: obtaining histories, physical exams; ordering and interpreting appropriate lab and x-ray studies, establishing diagnosis and initiating treatment plan.

·         Provides client education to promote wellness and support disease management process.

·         Prescribes and dispenses medications.

·         Document accurately all assessments, interventions, client/family responses, medication dispensed or prescribed, procedures, treatments and other data in medical record.

·         Communicates test results and follow-up to client.

·         Provides or participates in education for the client and family by assessing learning needs, formulating a teaching plan and initiating a teaching plan to meet the needs of the client.

·         Coordinates client care by triaging phone calls regarding urgency of appointments, and schedules hospital admissions and testing.

·         Maintains knowledge of available community resources.

·         Uses all available resources for diagnosis and treatment in an appropriate and cost efficient manner.

·         Maintains a clean, safe and efficient environment for clients, visitors, and staff.

·         Additional responsibilities may include hospital rounds and on-call coverage.

·         Completes all other assigned projects and duties.

·         Completes appropriate correspondence, phone calls and dictated letters to patients and referring physicians.

·         Check medical records box for lab slips and records in a timely fashion.

·         Carries out medical administrative functions necessary for quality patient care and compliance with accreditation requirements. 

         Participates in committee activities and has responsibilities commensurate with experience. May participate in Quality Assurance and Risk Management activities and committee work in assigned areas. 

        Attends department meetings as assigned
