Regional Social Worker

Job Summary:

Responsibilities:

  • Provide social work services to patients and residents across multiple facilities, including assessments, counseling, and psychotherapy

  • Develop and implement individualized care plans to address patients' and residents' social and emotional needs

  • Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication

  • Conduct social histories, identify risk factors, and develop strategies to mitigate risks

  • Provide crisis intervention and counseling services as needed

  • Maintain accurate and timely records of patient and resident interactions and treatment plans

  • Stay up-to-date with industry trends, best practices, and regulatory requirements

Requirements:

  • Bachelor's or Master's degree in Social Work (MSW) from an accredited program

  • Possess a valid social work license in the state of New Jersey LCSW

  • Minimum 3 years of experience as a social worker in a healthcare setting

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment with multiple priorities

  • Familiarity with electronic medical records (EMRs) and Microsoft Office Suite
