Job Summary: 

As a Participant Enrollment Specialist, you�ll help guide seniors and their families through the first steps of joining our program. You�ll build relationships, complete assessments, coordinate documentation, and support each prospective participant through the enrollment process. This position is ideal for someone who is compassionate, organized, and enjoys helping others while managing multiple moving parts. You�ll work closely with participants, families, community partners, and our Interdisciplinary Team to ensure a smooth and supportive enrollment experience.

You�ll be part of a caring, mission-driven team where communication, teamwork, and participant support come first.

Schedule: 

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties and Responsibilities: 

  • Meet with potential participants and families to explain the program and assess needs.
  • Complete in-home and community-based assessments for prospective Participants.
  • Coordinate required evaluations, documentation, and intake steps.
  • Attend intake appointments and support Participants throughout the enrollment process.
  • Complete intake and enrollment packets accurately and on time.
  • Serve as the main point of contact for referrals and guide them through each stage of enrollment.
  • Present referrals to the Interdisciplinary Team for review and ongoing collaboration.
  • Maintain accurate records and communication updates in the CRM and EMR.
  • Work closely with the Medicaid Enrollment Specialist to support a smooth transition to Medicaid processing and verification.
  • Participate in team meetings, training, and quality improvement activities.
  • Track weekly and monthly goals, enrollment progress, and key performance metrics.
  • Help identify barriers to enrollment and work with the team to create solutions.
  • Support additional projects and responsibilities as assigned.

Education and Experience: 

  • Bachelor�s degree preferred.
  • Experience working with frail or elderly populations is a plus but not required.
  • Knowledge of Medicare and Medicaid regulations is helpful.
  • Experience with community outreach, enrollment, sales, or marketing is a plus.

Physical Requirements:

  • Must be able to move intermittently throughout the workday.
  • Must be able to sit, stand, bend, and move during working hours.
  • Must be able to lift, carry, and support participants when needed.
  • Must be able to manage frequent interruptions and changing priorities.
