| Position Summary |
The Advanced Practice Registered Nurse (APRN) provides primary medical management in collaboration with the PACE physician(s) and/or Interdisciplinary Team. Conduct initial, semi-annual, and unscheduled assessments and reassessments; perform additional physical assessments of participants as needed; participate in the development and implementation of participant plans of care as a member of the IDT (Interdisciplinary Team). Evaluate participant physical complaint and provide appropriate treatment. Educate participants and families as needed regarding self-care and specific medical conditions. Function as a member of the interdisciplinary team. Provide daily oversight of the PACE Place Clinic. Regularly inform the interdisciplinary team of the medical condition of each participant, remaining alert to pertinent input from other team members, participants, and caregivers. Document care in the participant’s medical record consistent with documentation policies. The APRN ensures continuous communication with team members and maintains accurate documentation, contributing to a coordinated, high-quality care environment. | Essential Functions |
- Deliver primary medical care and collaborate with PACE physicians as needed.
- Perform initial, semi-annual, and unscheduled assessments and reassessments.
- Participate in developing and implementing individualized plans of care in coordination with the IDT.
- Conduct regular rounds in contracted institutional settings (e.g., assisted living and long-term care facilities).
- Manage acute and chronic conditions, provide preventive care, and prescribe medications and treatments in accordance with clinical protocols.
- Refer participants to medical specialists as clinically indicated.
- Document care delivery in compliance with organizational standards.
- Attend and contribute to interdisciplinary meetings and assist in 24-hour care planning.
- Support staff education and training related to clinical practices and quality improvement.
- Participate in policy development, standards of care, and clinical protocol reviews.
- Respond to participant health needs in various settings including home and hospital visits as needed.
- Participate in peer reviews, in-service meetings, and quality improvement initiatives.
- Maintain confidentiality, adhere to infection control protocols, and follow organizational safety procedures.
- Comply with all state and/or federal regulations to ensure:
- Reasonable and timely access to specialists as indicated by participant’s health conditions and consistent with current clinical practice guidelines.
- Provision of services are documented, tracked, and monitored across all care settings.
- Oversight of participants’ use of medical specialists and/or inpatient care.
- Required assessments are conducted in response to each service determination request.
- The organization is performing assessments, collecting and analyzing the data from these assessments to take actions to improve the quality of the participant’s life.
- Unscheduled reassessments are being conducted as required.
- Orders are based on the participant’s current needs and/or clinical practice guidelines.
- The IDT coordinates 24-hour care delivery that meets the needs of each participant across all care settings.
- That all ordered, approved, or care planned services are provided to the participant.
- All complaints regarding quality of care or service delivery are properly processed as a grievance.
- All individual issues within a grievance are reviewed, considered, and resolved.
- Immediately approved SDRs have notification of the approval during the same encounter and/or interaction.
- All participant medical records are complete, accurate and accessible to all staff.
- Only act within the scope of his or her authority to practice
- Perform other related duties as assigned.

| Qualifications |

| Knowledge and Critical Skills |
- Clinical proficiency in geriatric and adult care
- Strong knowledge of primary care, chronic disease management, and preventive medicine
- Ability to work collaboratively within an interdisciplinary team
- Skilled in clinical documentation, critical thinking, and problem-solving
- Effective verbal and written communication skills
- Understanding of regulatory standards for care coordination and quality assurance

| Experience |
- Minimum of 2 years of clinical experience working with frail or elderly
|
| License / Certification |
- Active RN and APN Licenses (Florida Board of Nursing)
- Nurse Practitioner Certification with prescriptive authority
- Current DEA license
- Current driver’s license and proof of auto insurance
- Basic Life Support Certification must be obtained prior to assuming the role, and must be updated prior to expiry thereafter

- Graduate of an accredited school of nursing
- Advanced Practice Nursing education with Nurse Practitioner certification
|

- Medical clearance for communicable diseases and up-to-date immunizations
- Completion of position-specific competencies before initiating participant care
- Availability to work non-standard hours including evenings, weekends, and holidays as needed
- Participation in continuing education and professional development activities
