How You Will Contribute:

 

Position Summary

The Licensed Practical Nurse (LPN) interacts with the clients and team members throughout their shift, providing high quality client-centered care within a specialized residential behavioral healthcare setting. The LPN collaborates with team members to ensure a safe and therapeutic environment is maintained within the treatment facilities. Position works alongside the Registered Nurse to carry out physician orders, prepare, and administer medications, assess, and document client conditions, and facilitate outside diagnostic or medical services that are ordered by the physician. 

 

Relationships and Contacts

Within the organization:  Maintains frequent and close working relationships with peers, immediate supervisor, and leadership throughout the facility.

Outside the organization: May initiate and maintain working relationships with clients, third party providers, pharmacies/suppliers, labs, and vendors, as needed.

 

Position Responsibilities

 

Essential Responsibilities

  1. Assists Registered Nurses, Nurse Practitioners and Physicians in identifying client needs, problems, capabilities, and limitations.
  2. Communicates observations with other clinical team members and makes recommendations for plan of care, providing input into the treatment team process.
  3. Identifies abnormal diagnostic data and brings to the attending physician’s attention.
  4. Documents the administration of medication in according to established policies and procedures.
  5. Coordinates all pharmacy and laboratory orders, delivery, and refills with the appropriate vendors.
  6. Ensures controlled medications are secured, monitors the administration and accurate recording of use.
  7. Administers medications according to physician orders and medication administration record.
  8. Administers injections including Tuber sol PPD reading, Long-Acting Antipsychotic injectables, other Intramuscular Injections, as ordered.
  9. Collaborates with team members to maintain a therapeutic environment in collaboration with other staff.
  10. Documentation reflects ongoing assessments of client’s behaviors and feelings as expressed.
  11. Prepares clients for discharge through individualized counseling and instruction prior to discharge in a clear and concise manner; providing written instructions if needed.
  12. Provides education to residents and families about medications, side effects, contraindications, and physical symptoms through individual counseling and group counseling.
  13. Provides review of clinical records for accuracy and quality documentation.
  14. Assists in the coordination of admissions, transfers, and discharges to provide a smooth and efficient flow of communication.
  15. Supports and adheres to facility infection control program.

  4. Adheres to facility policies, procedures, rules, and regulations, implementing regularly.
  5. Maintains positive attitude and acts as a team player with others on the treatment team.
