Lead Intake Coordinator - Home Care (LHCSA)

The Lead Intake Coordinator is responsible for overseeing daily intake and coordination functions, ensuring timely processing of referrals, efficient staffing support, and clear communication with caregivers, families, and payers. This role serves as a key operational lead, supporting workflow efficiency, service continuity, and compliance.

Key Responsibilities

• Oversee daily intake and coordination activities

• Support referral processing, authorizations, and start-of-care coordination

• Guide and support coordinators to ensure timely follow-up and task completion

• Communicate effectively with MLTCs, caregivers, families, and internal teams

• Assist with workflow improvements to reduce delays and lost referrals

• Ensure documentation and processes align with NYS DOH and payer requirements

Qualifications

• 2+ years of experience in home care intake and/or coordination (LHCSA preferred)

• Strong understanding of MLTC processes and home care operations

• Excellent organizational, communication, and leadership skills

• Experience with EMR systems and payer portals
