POSITION DESCRIPTION

This position is located at Green Mountain Armory, Camp Johnson, Colchester, VT. We are seeking a highly qualified Registered Nurse to serve as the Lead Non-Clinical Nurse Case Manager for the Vermont Army National Guard (VTARNG). This is a Key Personnel position responsible for promoting fitness and personal wellness for Service Members to ensure operational readiness. The Lead Case Manager will coordinate all aspects of care, ensuring proper utilization of resources and providing proactive interventions across various healthcare settings. Schedule of work consists of Monday - Friday, 0730 - 1630. Weekend support for Soldier Readiness events is required and telework is authorized as permitted by the Government. This position is subject to the Service Contract Labor Standards. The equivalent federal grade for this role is Registered Nurse III with an estimated monetary wage of $46.25 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUY ARE NOT LIMITED TO:

  • Act as the primary liaison and coordinate with the lead medical provider on the most complex case files on a bi-weekly basis.
  • Facilitate case management initiatives and deployment operations within 15 days of receipt.
  • Review MEDCOM policies and SOPs to ensure adherence to current standards and initiate corrective actions via Performance Improvement Plans when necessary.
  • Verify the accuracy of MEDPROS data prior to Medical Readiness Events to identify non-deployable Soldiers early.
  • Conduct monthly case reviews with VTARNG Commands and Health Services representatives.
  • Serve as the designated OPSEC Coordinator for the contract, obtaining Level II certification within 90 days of appointment.
  • Coordinate Soldier treatment plans with medical facilities and track compliance versus noncompliance during monthly reviews.
  • Maintain eCase profiles for referred Soldiers, ensuring profiles are established within one workday of provider referral.
  • Provide all medical and administrative charting in systems including eCase, MEDPROS, MEDCHART, and AHLTA within three business days of receiving information.
  • Prepare Soldier records for and attend Medical Evaluation Boards (MEB) and Physical Evaluation Boards (PEB).
  • Educate Soldiers on privacy rights (HIPAA), available resources, and their responsibilities regarding medical documentation and appointment attendance.
  • Generate monthly reports on case status, profile management (initiated, extended, closed), and adjudicated referrals.

REQUIRED SKILLS AND EXPERIENCE:

  • Must be a Registered Nurse (RN) with an active, unencumbered state license in the State of Vermont.
  • Familiarity with Individual Medical Readiness (IMR) and Army Regulations (AR 40-35, 40-66, 40-501, 40-502).
  • Ability to interpret rules, regulations, and requirements.
  • Proficiency with Army National Guard approved Microsoft Office 365 applications, including Teams, Word, Excel, and PowerPoint, with the ability to produce reports, spreadsheets, presentations, and collaborative documents in compliance with Government standards.
  • Excellent written and verbal communication skills.
  • Strong organizational and analytical thinking skills with attention to detail.
  • Ability to work both independently and collaboratively in high-demand environments.
  • Must be able to pass a favorably adjudicated Tier 1 investigation (or equivalent/higher).
  • Must be eligible for and maintain a Department of Defense Common Access Card (CAC).
  • Must complete required training within 30 days of hire.

REQUIRED EDUCATION:

  • Bachelor of Science in Nursing (BSN) preferred.
