Schedule: Full-Time | Monday–Friday, 9:00 AM – 5:00 PM
Salary: $34–$36 per hour

About the Role

Nassau Rehabilitation and Nursing Center is seeking a compassionate and dedicated Social Worker to join our team. In this role, you’ll play a vital part in enhancing residents’ quality of life by addressing their social, emotional, and psychological needs while supporting families throughout their care journey.

Key Responsibilities

• Conduct interviews with residents and families to develop personalized care plans

• Collaborate with families to promote inclusivity, independence, and well-being

• Assist with discharge planning and provide solutions for resident needs, including financial resources

• Work closely with interdisciplinary teams to ensure holistic, resident-centered care

• Advocate for residents by addressing social and emotional concerns and maintaining clear communication

Requirements

• Minimum of 2 years of experience as a Social Worker in a Skilled Nursing Facility (SNF) required

• Strong communication and interpersonal skills

• Compassionate, organized, and resident-focused

• Experience with Sigma EMR system preferred
