The TxHmL Support Associate collaborates seamlessly within DS Services, communicates proactively, and has knowledge of available community services. They update the Person Directed Plan (PDP) during Service Coordinator (SC) changes, notify the person and fax the updated PDP to the provider and LAR. They perform data entry in TMHP and CARE, reporting concerns to the supervisor and team members, including the Program Manager, and actively seek solutions to identified problems.

Defined Experience: Clerical, Secretarial, and Human Services
