Responsibilities:

• Supervise and coordinate the activities of caregivers and other staff members on weekends, ensuring that all residents receive quality care and services
• Develop and implement individualized care plans in collaboration with the healthcare team
• Conduct regular rounds to monitor resident care and safety, addressing any concerns or issues promptly
• Participate in quality assurance and quality improvement initiatives to ensure the highest standards of care
• Collaborate with other departments to ensure effective communication and seamless care transitions
• Provide guidance and mentorship to caregivers, promoting a positive and respectful work environment
• Maintain accurate and up-to-date records of resident care and services
• Identify and report any potential issues or concerns to management in a timely manner

Requirements:

• High school diploma required; degree in a related field preferred
• At least 0 years of experience in a healthcare setting, with a focus on long-term care
• Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment
• Ability to prioritize tasks and manage multiple responsibilities simultaneously
