JOB LOCATION : Main Campus
DEPARTMENT: OUTPATIENT CASE MANAGEMENT
SCHEDULE: Full Time, 8 HR Day Shift, 8am - 4:30pm
POSITION SUMMARY
The RN Outpatient Case Manager will serve as a bridge between the healthcare professional in the hospital setting and the patient and their caregiver. In the role, the Nurse Outpatient Case Manager will provide information and guidance to the patient and caregiver for an effective care transition, improved self-management skills and improved patient-physician communication. The Outpatient Case Manager will function as a guide for patients and caregivers, addressing critical issues and self-management tasks rather than providing direct care. The goal of the Outpatient Case Management Registered Nurse RN is to improve care transition by providing patients and caregivers with education, identification of referrals to other needed healthcare resources and support that promotes knowledge and self-management as they transition from hospital to home and other post-acute care.
KNOWLEDGE, SKILLS & ABILITIES
EDUCATION
Graduation from an accredited school of nursing
EXPERIENCE
Must have 5 years of experience as an RN.
KNOWLEDGE, SKILLS, AND ABILITIES · Ability to think critically and analytically with little or no supervision
· Ability to work effectively in situations of high stress and conflict and communicate the goals and outcomes of case management.
· Possesses strong clinical knowledge/expertise
· Ability to process information and prioritize
· Possesses exceptional verbal and written communication skills
· Possesses independent work habits, is self-reliant and self-directed
· Ability to learn, adapt, and change as required by the job functions
· Ability to maintain absolute confidentiality of material and information accessed and reviewed
· Basic computer literacy
· Ability to maintain attendance to meet standard job practices
Preferred Qualifications
· Experience in care coordination, case management, discharge planning, and utilization review
· Case Management certification
· Experience with Epic
WORKING CONDITIONS - ADA INFORMATION
· Ability to move freely, reach, bend, and complete light lifting
· Ability to use good body mechanics while performing daily job functions and ability to follow specific OSHA guidelines
