We are seeking a compassionate and dependable PRN Nights & Weekend On-Call Hospice Clinician to provide high-quality, patient-centered care to individuals receiving hospice services. This role is responsible for responding to after-hours calls, providing in-home visits as needed, and supporting patients and families during critical moments.

Key Responsibilities

• Respond promptly to after-hours and weekend on-call requests
• Provide skilled nursing care and symptom management in patient homes or facilities
• Perform assessments and interventions to ensure patient comfort and safety
• Communicate effectively with patients, families, and interdisciplinary team members
• Document all patient interactions in a timely and accurate manner
• Provide emotional support and education to patients and caregivers

Qualifications

• Active and unrestricted RN license in the state of MO
• Minimum of  2 years of clinical experience (hospice or home health preferred)
• Strong assessment and critical thinking skills
• Ability to work independently and make sound clinical decisions
• Reliable transportation and valid driver’s license
• Current CPR certification

Preferred Qualifications

• Previous hospice or palliative care experience
• Experience with electronic medical records (EMR)
• Comfort with end-of-life care and family support
