Licensed Vocational Nurse (LVN) - Home Health

Status: PRN

    Compensation Package

    Competitive piece-rate pay with multiple bonus opportunities:

    • Per Visit: $40.00 – $45.00 per completed visit (good-faith estimate; varies by visit type)
    • Mileage Reimbursement: $0.70 per mile
    • Drive Time: $25.00 per hour
    • Productivity Bonus: Earn extra incentives based on performance
    • Documentation Bonus: Reward for timely and accurate documentation

    Job Summary

    The Licensed Vocational Nurse (LVN) provides high-quality patient care in the home setting by following the established plan of care under the supervision of the Registered Nurse. This role is ideal for LVNs who enjoy one-on-one care, patient education, and helping individuals recover after orthopedic and surgical procedures.

    Key Responsibilities

    • Deliver nursing care as directed by the RN and patient plan of care
    • Provide accurate, timely, and compliant documentation
    • Collect assessment data and communicate findings to RN and physicians
    • Collaborate with the interdisciplinary care team
    • Use clinical equipment safely and efficiently
    • Engage in ongoing professional development

    Requirements

    • Active California LVN License
    • Current CPR Certification
    • 2+ years of direct patient care experience (home health experience required)
    • Valid driver’s license, reliable insured vehicle, and good driving record
