Job Description

$19.25+ an hour (with experience)

Key Responsibilities


  • Conduct patient and family assessments to identify needs such as food, housing, transportation, financial stability, and access to healthcare

  • Identify gaps in care, including missed primary care visits, preventative care needs, and inappropriate ER use

  • Document assessments and interactions in the patient record

  • Coordinate outreach efforts using phone calls, text messaging, mailings, home visits, and community events

  • Encourage and support patients in managing chronic conditions and engaging in preventative care

  • Collaborate closely with primary care providers, care managers, and support staff to improve patient outcomes

  • Address barriers to care and assist patients in accessing timely services

  • Educate patients on available resources, including financial assistance, insurance programs, and sliding fee discounts

  • Connect patients with community services and support programs

  • Build and maintain partnerships with community organizations

Qualifications


  • Completion of Community Health Worker (CHW) Training required
  • Seasonal Flu Vaccine    

  • Experience working with diverse populations and adapting communication styles as needed
