What You’ll Do:

 

  • Provide a wide scope of activities for Developmental Disabilities Support Coordination services for a two-county area.
  • Work closely with program staff to provide initial and ongoing training.
  • Audits the accuracy and timeliness of ISPs, UR requests, case notes, budget, and other documents.
  • Attends interdisciplinary team meetings to discuss client cases in relation to eligibility, support plans, progress, and possible changes in the service program
  • Conduct pre-survey reviews to identify and correct deficiencies before SMH/CMS/MMAC reviews.
  • Assist Program Managers in gathering data for reviews.
  • Develop and maintain flowcharts and spreadsheets, tracking data and reporting findings to the team.
  • As needed, manage a selected caseload of clients with intellectual disabilities/developmental disabilities in placement or natural homes.
  • Performs other related duties as assigned.

Requirements

Minimum Qualifications

 

Required:

  • Bachelor’s degree in social work (BSW) or equivalent human services degree; OR Registered Nurse (licensed)
  • Two (2) years of experience in case management in a human service field.
  • This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.

 

Preferred:

  • Previous targeted case management experience.
