Job Description

The Social Worker helps maintain the
psycho-social welfare of all residents and assists in completing the
admissions/discharges for New Glarus Home in the skilled nursing facility. We
are licensed for 100 residents and the successful individual in this position
will have a strong passion for working in Long Term Care and will be deeply
committed to providing exceptional levels of Care and service to our residents
and their families.

 

Essential Job Functions:

Assist with the following-

  • Complete admission of residents: conduct
    pre-admission interviews, prepare admission packets, write social history
    of resident, welcome resident upon arrival
  • Counsel residents and their families; Ongoing
    education to residents and their families
  • Develop and implement psycho-social care plans;
    review quarterly
  • Complete MDS's documentation
  • Make referrals and recommendations to other
    agencies involved with the residents
  • Consult with staff regarding social and emotional
    factors related to resident's health problems
  • Coordinate transportation for residents to special
    appointments and services with the assistance of the Unit Clerk.
  • Record plans, goals, and progress on individual
    residents
  • Provide in-service training on Resident Rights and
    Quality of Life issues to nursing home staff
  • Assist with discharge plans and home visits as
    needed
  • Coordinate resident/family/staff MDS and care plan
    meetings
  • Participate in all Medicare discussions and
    implement denial notifications
  • Assist resident and family with financial need
    referrals
  • Coordinate with other discipline
    policies/procedures of quality assurance
  • Oversee resident council and family council groups
  • Coordinate and conduct tour of campus/facilities
  • Coordinate all discharges

 

Requirements:

 

Educational/Experience:

  • High School Diploma
  • A minimum of 2 years experience in Social Services
    in a hospital, long term care facility or other related health care
    facility preferred

 

Licensing:

  • Valid drivers license
