Compensation and Benefits:

• Earning potential: Fee for Service Position (1099)


• Flexible Schedule: 1 to 2 days a week to assist in Palliative care in a Facility Setting


• Malpractice insurance provided


• License reimbursement and CEU support


• Paid training and onboarding


• Ongoing education and clinical support, including physician-level consult access


• Autonomy with Support to deliver independent care with access to physician-level consults.

 

Key Responsibilities

• Deliver person-centered care using evidence-based practices


• Collaborate with interdisciplinary facility teams


• Conduct assessments, develop treatment plans, and manage follow-up care


• Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)


• Educate patients, families, and staff


• Complete timely documentation (within 24 hours)


• Compliance with facility, state and federal regulations/protocols.
