PRIMARY RESPONSIBILITIES
The Social Worker/Clinical Therapist will be responsible for working with the patients overseeing and assisting in the admission process, identification of needs and problems, makes referrals to community resources, and ensuring the mental and social health of each resident.
- Develop relationships with residents and their responsibility party as needed for admissions, counseling, and identifying needs.
- Complete acute care plans as needed and attend care plan meetings
- Discuss advance care planning with the party responsible upon admission and as per state regulations and complete necessary documents.
- Provide staff with any important department updates.
- Perform suicidal/homicidal assessments and required follow up. Assist in investigation as requested by Administrator.
PHYSICAL REQUIREMENTS
- Constant use of speech to share information through oral communication.
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, reaching and keyboard use/data entry.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of smell to detect/recognize odors.
- Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Occasional lifting/moving of patients.
- Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
Education & Requirements
- Master's in social work or Mental Health Counseling LCSW or LMHC
- Must be licensed in the State of License
- Previous experience working in Acute Care, or Skilled Nursing.
- Ability to Communicate in English
- BLS required
