What You’ll Do

Serve as the primary point of contact for hospital referrals, working closely with case managers, discharge planners, and healthcare partners

Review clinical documentation and determine appropriateness for skilled nursing admission

Coordinate seamless transitions from hospital to post-acute care settings

Build and maintain strong referral relationships to drive census growth

Partner with internal clinical and admissions teams to ensure timely, appropriate placements

Identify new referral sources and support ongoing marketing and outreach efforts

Represent the organization at hospital meetings, networking events, and community outreach

What You Bring

Active RN or LPN license (required)

Experience in skilled nursing, case management, admissions, or healthcare marketing preferred

Strong clinical assessment and decision-making skills

Ability to build relationships and influence referral partners

Highly organized with the ability to manage multiple priorities
