ABOUT THE UNIT:
The Medication History Technician performs medication histories for new hospital admissions under the direct supervision of a clinical pharmacist. They will document medication histories in the electronic health record in an accurate and timely manner, providing complete and accurate information for each medication (i.e. drug name, route, frequency, indication, and date/time last dose was taken). They utilize available resources for medication history collection including but not limited to: patient/family interview, outside facility medication administration records, outpatient pharmacy records, and physician office records. They will verify/update patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record. They will assist the pharmacist by identifying patients in need of discharge counseling, preparing education materials for the patient as necessary. In addition to specific job functions, they will perform other routine pharmacy technician job duties as assigned.
JOB REQUIREMENTS:
- PTCB Medication History Certificate Preferred - Required within 1 year of hire
- Obtain a minimum of 3 hours of continuing education related to the medication history process annually - Required
- 3 Years Hospital or Institutional Practice Experience - Required
