Position Summary:
Assists the patients of the organization to overcome seriocomic barriers to achieving their health and wellness goals. Ensures access to quality healthcare for all individuals by facilitating access to community resources, referral services and life skill education for patients and their support systems.
Qualifications:
Education:
Associate degree in social services, Bachelor’s degree a plus
Years of Experience:
Two (2) or more years in social services or related field
Knowledge, Skills and Abilities:
- Knowledge of community resources relevant to social detriments of health
- Strong verbal communication skills, including excellent active listening skills
- Excellent written communication skills
- Ability to work harmoniously as part of a health care team
- Ability to meet approved minimal physical and medical standards
- Fluency in Spanish is a plus
Scope of Responsibilities:
- Provide individual and family supportive counseling and referral to help improve personal and social functioning and resolve emotional and psychological problems
- Provide advocacy assistance in obtaining necessary services
- Partners with the Community Health Educator and other agency representatives to identify and work to address unmet community needs
- Partner with community agencies to facilitate mutual referrals.
- Provide documentation in a timely manner according to department standards
- Model skillful, supportive, strengths-based interactions with patients and co-workers
- Maintain confidentiality of information about patients and their families
- Perform other duties as directed by supervisor
- Document all patient contacts, case updates, and referrals according to established protocol and standards
- Maintain statistical data necessary for reporting requirements
- Track appointment compliance for care-managed patients who have missed appointments, and document outcomes
- Prepare reports and audits requested by supervisor

Work environment:
- Primary work environment is a busy multi-site, multi-cultural ambulatory care setting. There is routine exposure to infectious disease and blood borne pathogens.
