Generations Family Health Center, Inc. is a regional community health center covering a 37-town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich, and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental, and behavioral healthcare, as well as care coordination and community outreach.
Location: Putnam / Willimantic, CT
Position: The Registered Dietitian provides high-quality, culturally competent nutrition services and diabetes education to Generations Family Health Center patients. This role includes individualized counseling, teaching self-management classes and assistance with disease specific programs including, but not limited to, Healthy Weight Clinic and Diabetes Self-Management Education and Support. The Registered Dietitian also contributes to the development of evidence-based nutrition interventions, maintains accurate documentation, and promotes nutrition services across the organization.
Responsibilities:
- Nutrition counseling tailored to each patient’s medical needs, lifestyle, and cultural food practices with sensitivity to the diverse populations served.
- Teach self-management classes focused on diabetes, weight management, and other disease-specific nutrition needs.
- Assist in developing diabetes prevention programming, diabetes support programming, and pediatric healthy weight programing.
- Maintain accurate, timely documentation of all nutrition encounters in the E.M.R. including billing requirements.
- Collaborate effectively with interdisciplinary team.
- Promote nutrition services across Generations Family Health Center sites and participate in community outreach and engagement collaboration activities as needed.
- Maintain an active Registered Dietitian status through ongoing CEU participation and adherence to professional standards.
- Conducts comprehensive nutrition assessments for patients to identify nutrition-related needs, risks, and appropriate interventions. Collects and evaluates medical, dietary, psychosocial, and lifestyle data, including review of medical records, laboratory values, and patient history.
- Participate in peer review process to support quality improvement and professional growth.
- Perform additional duties as assigned in support of department and organizational goals.
