Job Description

Join Our Team:
We are seeking experienced and empathetic Hospice RN Case Managers to join our dedicated Scottsdale team. In this role, you will be essential in delivering high-quality hospice care and guiding patients and families through their journey with dignity and compassion.


Positions Available:

  • 2 Full-Time RN Case Managers
    • 1 covering the West Valley
    • 1 covering the East Valley


Key Responsibilities:

  • Manage and coordinate care for a caseload of hospice patients.
  • Develop, implement, and update individualized care plans.
  • Provide skilled nursing visits, including symptom management and end-of-life care.
  • Educate patients, families, and caregivers on hospice philosophy and care expectations.
  • Collaborate with an interdisciplinary team, including physicians, social workers, chaplains, and aides.
  • Ensure accurate, timely documentation according to regulatory and company standards.
  • Participate in on-call rotation as needed.



Qualifications:

  • Current, unrestricted RN license in Arizona.
  • Minimum of 1 year hospice experience preferred.
  • Strong assessment, critical thinking, and communication skills.
  • Compassionate, empathetic, and committed to high-quality end-of-life care.
  • Reliable transportation, valid driver’s license, and insurance.
