Job Description

We are looking for a compassionate and committed Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing team. We partner with nursing homes throughout Rochester NY, delivering high-quality behavioral health services to older adults. This role is ideal for a clinician dedicated to geriatric psychiatry and passionate about enhancing the emotional and psychological well-being of senior residents.



Requirements

Key Duties

  • Perform thorough psychiatric assessments and mental health evaluations for residents in long-term care facilities.

  • Identify psychiatric conditions and create customized treatment plans that meet each patient’s specific needs.

  • Manage psychiatric medications, monitor progress, and adjust interventions when clinically appropriate.

  • Work closely with an interdisciplinary team—including physicians, psychologists, nurses, and social workers—to ensure coordinated, comprehensive care.

  • Provide therapeutic support, including counseling and psychotherapy when needed.

  • Educate patients and families regarding diagnoses, treatment options, medication use, and strategies to support mental wellness.

  • Maintain complete, timely, and compliant clinical documentation within the electronic health record system.

  • Participate in team meetings, case reviews, and quality improvement initiatives that support patient outcomes and program growth


Qualifications
  • Active New York State Nurse Practitioner license with a Psychiatric–Mental Health Nurse Practitioner (PMHNP) certification.

  • Ability to work autonomously across multiple facilities while effectively managing a patient caseload.

  • Solid clinical judgment with strong diagnostic abilities and knowledge of psychiatric treatment methods.

  • Excellent communication and interpersonal skills; able to build trust with patients, families, and staff.

  • Comfortable using EHR platforms and basic computer applications.

  • A compassionate, patient-first approach to care.
