What You’ll Do

• Perform start-of-care visits and ongoing patient assessments
• Develop and manage individualized care plans
• Provide hands-on nursing care, education, and emotional support
• Monitor patient progress and adjust care as needed
• Collaborate with physicians and interdisciplinary teams
• Supervise LVNs/LPNs and Home Health Aides
• Ensure accurate, timely documentation (Homecare Homebase preferred)
• Build strong relationships with patients and families

What You’ll Need

• Active RN license
• Valid driver’s license, auto insurance & reliable transportation
• CPR certification
• 2+ years of RN experience (home health preferred)
• Strong clinical judgment and communication skills
