Job Description
The Program Coordinator provides strategic leadership, oversight, and coordination of program services to ensure delivery of high-quality care and full compliance with all applicable regulations. This role supervises Program Managers, staff, and maintains a client caseload. The Program Coordinator is responsible for ensuring effective, efficient service delivery across all programs while upholding organizational policies, procedures, and regulatory standards.

Job Details:
  • Full-Time position with standard office hours (9am-5pm) and occasional flexibility to meet client needs. This position also provides on-call coverage as needed to ensure appropriate staffing/coverage.

  • Reports To: Executive Director of Health & Operations

Essential Duties and Responsibilities: 
  • Supervise, mentor, and support Program Managers and staff to ensure effective performance and professional development. 
  • Manage referrals, service coordination, 24-hour residential programs, while ensuring full compliance with organizational policies, procedures, and applicable state regulations. 
  • Perform all duties associated with the Program Coordinator role.  
  • Provide guidance and problem-solving support to staff regarding client care and service delivery.  
  • Ensure effective communication and coordination among clients, families, staff, and external partners.  
  • Participate in and lead meetings, including team meetings, care conferences, and organizational meetings.  
  • Monitor program operations to ensure quality standards and continuous improvement.  
  • Conduct assessments to determine clients’ needs for personal care and supportive services.  
  • Develop, implement, and monitor individualized service plans.  
  • Provide staff training as needed, onsite training, client specific training, personal care and challenging behavior training, etc. 
  • Maintain thorough knowledge of assigned clients and their care needs. 
  • Track staff scheduling ensuring proper shift coverage while monitoring attendance and overtime accordingly. 
  • Weekly “site visits” ensuring adequate care coordination, monitoring staff work performance, etc. 
  • Conduct Staff Performance Reviews during probation periods and annually.

 Qualifications: 
  • Strong leadership and managerial skills.  
  • Excellent verbal and written communication abilities.  
  • Strong organizational skills, including time management, multitasking, and follow-through.  
  • Proficiency preferred in platforms such as Microsoft 365, Care Smartz, Paychex Flex, Residex, etc.  
 
Education and Experience: 
  • Bachelor’s degree or a minimum of four years of relevant experience, or an equivalent combination of education, training, and professional experience that demonstrates the required knowledge and abilities.
