NorthPoint Health & Wellness Center is seeking a Nurse Practitioner to provide a full spectrum of family health care services in their community clinic, which is a diverse, urban, community health center in North Minneapolis whose mission is "Partnering to create a healthier community".

Location and hours:
This position is located at the NorthPoint Health & Wellness Center at 2220 Plymouth Avenue North, Minneapolis, MN 55411. Work hours for this position will be 20-40 hours per week, Monday through Friday days, with occasional coverage for evening and Saturday clinic hours. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

In this position, you will:
- Perform initial and on-going assessment of patients' medical, physical, and psychosocial status.
- Obtain and evaluate health and medical history, identify medical and health risks, and triage those who require physician consultation.
- Conduct preventative screening procedures and perform physical examinations based on age and history, complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow up, and develop client education plans.
- Make appropriate referrals to other health professionals and community agencies.
- Order, conduct, and interpret diagnostic tests, order appropriate therapeutic treatments, and prescribe, administer, and dispense therapeutic devices and legend drugs.
- Contribute to on-going medical and nursing staff teaching program, initiate and participate in research studies, and support new and existing program, which expand the organization's role in the community.
Need to have:
One of the following:
- Possession of a valid license as a Registered Nurse issued by the State of Minnesota and a National Certification as a Nurse Practitioner (CNP).
- Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.
Nice to have:
- Experience:
- As a Family Nurse Practitioner.
- Working with populations diverse in race, ethnicity, age, gender, identity, ability, and sexual orientation.
- Utilizing Epic or another similar electronic health records system.
- Ability to:
- Demonstrate both verbal and written communication skills.
- Demonstrate problem-solving skills and adapt to an ever-changing clinic practice environment.
- Respect, appreciate, and promote teamwork among diverse co-workers and patients regarding race, gender, disability, values, and lifestyle perspectives or interests.
