Job Description
Summary:
Contributes to overall goals of the department. Attend in-service offerings, workshops and seminars related to Physical Therapy. Update knowledge and skills through reading and consulting with other professionals.

Job Qualifications:
Education: Masters Degree in Physical Therapy

Licensure, Registry or Certification:
Current Texas License for Physical Therapy - MPT
Current Drivers license
Current CPR
Current TB test (annual retested)
Current Vehicle Insurance

Personal Job Related Skills:
Knowledge of equipment
Supervisory skills, effective oral and written communication skills.
Ability to assist physicians in evaluating all clients referred for Physical Therapy Evaluation.
Develop plan of therapy based on results of diagnostic procedures, diagnosis and prognosis and with approval of physician.
Ability to consult with patient/family when developing plan of care.
Ability to work with / consult with other departments in securing appropriate referrals to other services, physical therapy, speech therapy, social services, DME, etc.
Ability to perform an initial assessment and develop database for Home Health patients.

Experience:
Two (2) year of clinical experience in rehabilitation.

Essential Duties and Responsibilities:
Maintain inventory level of supplies
Insures department well stocked for necessary operation
Recommend purchases and supervise storage and control of supplies
Insures department equipment is in good working order
Keep abreast of new technology
Report equipment problems to maintenance

Outpatient Therapy:
Supervise and participate in outpatient treatments
Design and schedule treatment plan
Uses appropriate treatment modalities/procedures to accomplish therapy goals.
Instructs patients/family in appropriate techniques and procedures to meet therapy goals.
Maintain current progress notes on all patients. Complete written documentation or dictation on day of treatment.
Consult with physician regarding problems/progress of patients.
Supervise treatments conducted by the PTA to ensure compliance with treatment plan and document in the record.
Revises and updates treatment plan regularly.

Home Health Patients:
Provides Physical Therapy to patients in their place of residence.
Uses appropriate treatment modalities/procedures to accomplish therapy goals.
Instructs patients/family in appropriate techniques and procedures to meet therapy goals.
Maintain current progress notes on all patients. Complete written documentation or dictation on day of visit and submit progress notes at least weekly to agency.
Participates in case conferences for all patients.
Consult with physician regarding problems/progress of patients.
Supervise the COTA every 30 days to ensure compliance with POC and documents visits in the clinical record.
Notify the Home Health R.N. Case Manager of changes in patient progress and condition.
Revises treatment plan regularly.
Instruct other health care providers in appropriate procedure / techniques to achieve therapy goals
Willing and able to drive private automobile in all weather conditions.
Effective communication and working relationships with home health agency staff, patients, families, physicians and members of community.
Participate in committees that are required by regulations and hospital policies.
Presents positive professional image in all contacts.

In-patient / Swingbed Patients:
Provides Physical Therapy to patients in hospital room.
Uses appropriate treatment modalities/procedures to accomplish therapy goals.
Instructs patient in appropriate techniques and procedures to meet therapy goals.
Maintain current progress notes on all patients. Complete written documentation or dictation on day of treatment.
Participates in case conferences for all patients.
Consult with physician regarding problems/progress of patients.
Supervise the PTA every 30 days to ensure compliance with POC and documents visits in the clinical record.
Revises treatment plan regularly.
Instruct other health care providers in appropriate procedure / techniques to achieve therapy goals.
