Diagnostic:
1. Demonstrates knowledge of disease process and follows standard of care practice consistent with national and CHC/SEIA guidelines/recommendations.
a) History and assessment (promptness, thoroughness, significant negatives, problems oriented, other).
b) Physical examinations.
c) Laboratory use (test selection, sequencing, cost-effectiveness, etc.).
d) Use of diagnostic x-ray/other test modalities or test selections.
e) Comprehensive Care.
2. Completes chronic disease management within standard set by CHC/SEIA utilizing completed chronic disease documents.
3. Completed scheduled health maintenance series utilized.
4. Completes proper assessment, providing standards of care to include patient education and counseling and documents this on appropriate forms in the medical record.
Referrals:
1. Made to the appropriate consultant in a timely fashion.
2. Sensitive to patient and organization cost.
Documentation:
1. Maintains the standards of accurate and complete recording and reporting of patient information.
a) EM coding is accurate.
b) CPT and ICD-10 coding is accurate.
c) Completes charts and tasks in the timelines set forth in “Patient Health Information Documentation Policy”.
d) Hospital records (if provider sees patients in the hospital) are completed and signed per policy(ies).
Organizational Relations:
1. Supports a service-oriented atmosphere in accordance with CHC/SEIA mission and philosophy.
2. Participates in call rotation as expected, if applicable.
3. Coordinates with staff requests of CME, vacation, etc.
4. Participates in Peer Review Process
5. Follows infection control and safety procedures as outlined in CHC/SEIA policies.
6. Maintains all state and federal licensures and/or certifications required to provide physician services, including but not limited to state medical licensure; state-controlled substances certificate; federal DEA registration; etc.
