Description

The Peer / Patient Navigator works with the HIV/AIDS population, provides ongoing navigation and support, and provides linkages to needed services. This position can play a significant role in improving health outcomes, especially for patients living with complex chronic health or behavioral health conditions.

 

Requirements

KNOWLEDGE, EDUCATION, SKILLS, AND ABILITIES REQUIRED:

  • Experience working with the HIV/AIDS population
  • Peer Certification is required and can be completed while working in this program
  • Self-disciplined: reliable and demonstrably responsible
  • Articulate in presenting ideas and opinions
  • Supportive and demonstrably empathic in his relations to peers
  • Stable and drug-free
  • Exhibiting dialogical evidence of having dealt with issues of HIV infection and/or drug abuse

·           Proficiency in English and/or Spanish, both written and spoken language

·           Commitment to providing quality health care services to all

·           Demonstrated commitment to working with patients and staff from a wide range of ethnic, economic, cultural, and social background

·           Excellent communication skills

·           Professional demeanor

·           Excellent organizational skills

Summary

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Provide home and field-based services, including health education and other healthcare and social service encounters as required
  • Maintains patient/family confidentiality
  • Maintains close collaboration with HIV TEAM
  • Provides accompaniment to routine primary care appointments and other healthcare and social service encounters as deemed appropriate by Case Managers/Care Coordinators
  • Participates in-group counseling
  • Participates in in-reach and out-reach activities to include telephone calls, home visits and collaboration with Betances Health Center staff and other outside agency.
  • Provides home and field-based services
  • Health education counseling
  • Provides Direct Observed Therapy (DOT) and education on treatment adherence
  • Participates in planning/assessment activities
  • Provides ongoing navigation and support including assistance with insurance and other entitlements
  • Provides appointment reminders and linkages to medical and social services
  • Provides feedback to the Care Team based on his/her field observations 
  • Peers/Patient Navigators will participate in CQI activities
