What You Will Do  - Essential Functions

The Nurse Practitioner, Travel IHA conducts in-person and occasionally virtual, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members. This role is fully travel based across the state of New York, requiring overnight stays. The NP will perform comprehensive health evaluations inside members’ homes, identify risks, document findings accurately, and contribute to improved health outcomes through preventive and patient-centered care. 

Please note the position requires you to meet patients in their homes throughout Monroe County, NY and you will be expected to cover a territory radius of 50 miles from your home address.

  • In‑Home Clinical Care & Patient Engagement
    • Conduct comprehensive in‑home assessments, identifying risk factors, closing health gaps, and educating patients on screenings, disease management, and preventive care.
    • Demonstrate exceptional bedside manner and culturally sensitive care while collaborating with interdisciplinary teams to ensure coordinated, patient‑centered care.
  • Quality, Compliance & Clinical Standards
    •  Adhere to health plan requirements, regulatory guidelines, and organizational quality standards while maintaining accurate EMR documentation within required timeframes.
    • Uphold ethical practice, patient confidentiality, and participate in quality improvement initiatives to support safe and effective care delivery.
  • Mobility, Autonomy & Professional Practice
    •  Travel throughout New York State—including frequent overnights—to deliver care across diverse home and community settings.
    • Maintain reliability, professionalism, and sound clinical judgment with flexibility and autonomy in managing schedules and patient follow‑up needs.

Who You Are

Required Qualifications:

  • Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.

  • Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant

  • Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.

  • Active Drug Enforcement Administration (DEA) license.

  • Current Basic Life Support Certification for health care providers.

  • Strong clinical assessment and diagnostic skills.

  • Excellent communication and patient education abilities.

  • Proficiency in electronic medical record (EMR) systems.

  • Valid proof of sufficient car insurance coverage for the state that appointments are scheduled to be completed in with access to reliable transportation.

  • Ability to travel statewide and stay overnight as required.

  • Valid and active driver’s license for a minimum of 3 years without restrictions, and minimum of 7 years with no DUIs or other felony driving convictions.

  • NY State only: Minimum of 3600 hours of practice experience, or with 3 years of clinical experience (autonomous provider), in a primary care or longitudinal care setting

  • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Working conditions:

  • Ability to lift and carry up to 20 lb
  • Mental and physical stamina to perform assessments and documentation.
  • Work is performed in member homes and may involve exposure to varied conditions.
