Job Description

POSITION DESCRIPTION

Exciting opportunity for a seasoned psychotherapist to join our Wellness team located at The Nest Community Health Center. URAM-Harlem United’s Wellness Center is a Licensed State Operated Article 31 Mental Health Clinic Collocated in our Federally Qualified Health Center (F.Q.H.C.). Under the supervision of the Managing Clinical Director, the Psychotherapist will provide individual, group and family psychotherapy, supportive counseling, crisis intervention and care coordination for our Article 31 clinic

This position is full-time.


ESSENTIAL JOB FUNCTIONS

The following duties are mandatory requirements of the job:

• Screen/assess clients for appropriateness for admission into the Article 31 Mental Health Clinic in collaboration

with a psychiatrist/psychiatric nurse practitioner during initial visits.

• Utilize evidence-based tools (PHQ-9, GAD- 7, AUDIT, DAST, etc) and DSM-5 to help formulate an appropriate

clinical diagnosis for assigned clients.

• Knowledge and past utilization of interventions (psychodynamic, DBT, TF-CBT, MI and cognitive processing)

preferred, use of each intervention dependent on individual client.

• Hold a caseload of 70 patients, with 10-12 sessions daily

• Establishing treatment goals and objectives for and with clients, as part of the interdisciplinary care planning

process

• Assume role of primary psychotherapist for assigned clients, which includes documenting all sessions as well as

all communication with collaterals and shared providers.

• Participate in on-site crisis response

• Attend regular supervision with Managing Clinical Director and weekly team meetings

• Participate in on-site crisis response, including rapid assessment, de-escalation, and coordination with

multidisciplinary teams to ensure client and staff safety.

• Coordinate and facilitate case conferences for providers across agencies serving mutual clients, for high-risk

cases and following incidents.

• Ensure the integrity and confidentiality of all patient information in the EHR by not sharing personal username or

password with other staff.

• Maintain integrity and confidentiality of all patient information in the EHR by exiting the application or locking

the system when away from the computer.

• Complete timely, accurate, and comprehensive clinical documentation, including progress notes (72 hours),

assessments, treatment plans, and crisis interventions, in accordance with agency and regulatory standards.


EDUCATION, CERTIFICATION AND QUALIFICATIONS

• Masters in Social Work plus LMSW/LCSW with at least two years’ experience providing psychotherapy and

• Experience providing psychotherapy in an Article 31 or similar outpatient clinic required.

• Experience working with individuals with history of substance use, mental illness, chronic homelessness and/or

HIV/AIDS and other chronic health conditions required.

• Experience working with late adolescents, adults and families desired.

• Experience facilitating therapy groups desired

• Prefer candidates already credentialed with insurance panels.

• Bilingual in Spanish a plus.
